Who can become a member?
To become a member, an individual must be at least 18 years old and have an Axis 1 diagnosis. Most importantly, the individual must be willing to follow the rules and regulations of our community policy on safety and exercise common courtesies and considerations towards everyone. The person can be referred by the Community Mental Health Center through the Access Line 832-3100.

To prevent delays in the application/membership process, the following forms must be included; a current copy of a Master Recovery Plan, Insurance cards (e.g. Ohana, Evercare, Medicare, etc), Psychiatric Evaluation, and Tuberculosis Skin Test (PPD) card. Forms can be submitted by fax 675-0071 or dropped off at Waipahu Aloha Clubhouse 94-091 Waipio Point Access Road Waipahu, 96797. Application will be reviewed by the membership committee and potential member will be contacted.
